Number every fact. One sentence per number. First person, past tense, nothing you didn't witness. That structure lets a clerk cite '¶4' instead of hunting through prose — which is exactly why institutions ask for affidavits.
Frequently asked questions
What's the difference between the jurat here and an acknowledgment?
A jurat = you swore the contents are true (used for affidavits). An acknowledgment = you confirmed you signed voluntarily (used for letters and deeds). Notaries carry both stamps — the template tells them which you need.
Can affidavits be in other languages?
